Cardiac Rehabilitation 

The cardiac rehabilitation team consists of a Clinical Nurse Specialist (CNS) and a senior physiotherapist. We provide "Phase 1" Cardiac Rehabilitation as defined by the British Association of Cardiac Rehabilitation (BACR) in accordance with guidelines set by the British Heart Foundation (BHF). We try to see all acute myocardial infarcts (AMIs) and non-ST elevation MIs (NSTEMIs) as well as providing post-op advice/ information for all open heart surgical patients whilst in hospital. We also provide advice to cardiology patients undergoing elective stenting for ischaemic heart disease (IHD). We provide an out-reach telephone help line for patients discharged from Derriford who have queries about their condition and lifestyle modification advice. We liase with our community colleagues to provide them with current information about their patients on discharge from hospital and make patients aware of any community based Cardiac Rehabilitation available in their local PCT.

Any healthcare practitioner may refer a patient to the service providing the patient is:
1. aware of the diagnosis
2. mentally able to understand the information discussed
3. the event, in hospital, is an acute event
