Physiotherapy Self-Referral

You can access our physiotherapy service by filling out our online self-referral form. If you prefer you can also pick-up a paper copy of the form from your GP surgery and return it to us by post. View our Physiotherapy postal details.

To use our service:

  • You must be 16 years or older and registered with a Plymouth, Kingsbridge or Tavistock GP.

  • You must be seeking help with a Musculoskeletal problem, such as neck or back pain or soft tissue or joint problems such as strains and sprains.

  • You will only need to consult your GP if you have any of the following conditions: unexplained weight loss, unexplained bladder or bowel problems, history of cancer, night pain, fever or night sweats, unsteady on feet and pins and needles/numbness in both arms or in both legs.

  • You cannot self-refer if you have a neurological condition such as Multiple sclerosis, Parkinson's, Stroke and respiratory conditions. For treatment of these conditions, you would have to be referred by your GP in the normal way and to the appropriate Physiotherapy service.

 

Filling out the form

  • You can fill out the form for one condition. Once physiotherapy care has been completed for this you can fill out another form if needed. This is to keep waiting times down and improve the quality of your care.

  • Referrals will be assessed on a clinical basis to identify urgent cases vs routine

  • If your referral is not appropriate, we will send you a letter explaining why and you will be encouraged to contact your GP if needed.

  • It may take up to 28 working days to process your referral, and up to 18 weeks for you to receive an appointment.

If you have been told you may have one of the following musculoskeletal conditions

  • Plantar Fasciitis – usually pain under the heel that extends to the arch of the foot

  • Bunions or Hallux Abducto Valgus deformity

  • Achilles tendon pain- pain on the back of the heel or ankle

  • Metatarsalgia – general pain in the front half of the foot excluding the toes

  • Morton’s neuroma – pain in the front half of the foot including the toes.
