We are excited to be working on one of the biggest digital transformations we’ve ever undertaken - launching one single electronic patient record across Devon.
An electronic patient record (EPR) is the collection of patient health information in a digital format, that can be shared across different health care settings. It will include a range of data, including medication and allergies, immunisation status, laboratory test results, radiology images, vital signs and personal statistics like age and weight. It will also be the place where we book appointments, request tests and prescribe medicines.
The new Devon EPR will bring together all patient records across three partners:

For patients, one of the most exciting parts of the EPR is the new patient portal called MY CARE.
When the EPR launches in July 2026, MY CARE will bring your medical information and interactions with your clinical team into one place.*1 You will be able to access it using an app on your mobile phone or via your computer.

This means you don’t have to remember your medical history or repeat the same information to different members of staff, no matter where you are in Devon.
While we are working to configure the new system, MY CARE is not currently available, so you don’t need to do anything yet.
We’ll let you know when MY CARE is available for download, ready to use.
Using MY CARE is optional. So, if you prefer, you can continue receiving your health information as you do today.

What is an electronic patient record (EPR) system?
An EPR system is a digital platform that brings all your patient information, from medical history to results of investigations and medications prescribed, together in one place. Currently we hold your records in different places, such as paper-based notes and on several digital systems. Once we implement an EPR system, all your medical information will be kept together in one single electronic patient record. This means your clinician can see everything relating to your care in real time, helping to improve the quality of care we provide. It also means as a patient, you only need to give us your information once. The system we are introducing is called Epic.
Why have you chosen Epic?
Epic has a strong track record in collaborating with the NHS, having worked with many trusts to deliver their EPRs, including Royal Devon University Healthcare NHS Foundation Trust. Both Torbay and South Devon and Plymouth went through extensive procurement processes to determine which EPR would be right for the staff and patients in their areas.
Why are we introducing an electronic patient record (EPR) system?
Having access to all your information in one place will help improve the quality of care we provide. For example, clinicians will have a full picture of your medical history and treatment their fingertips, which will help inform and speed up decision-making. We can also run services and manage our hospitals more efficiently as your information is added to your record and available in real time. This means at any given point we can see where a patient is in our hospitals, what beds are free and when a patient may be ready to go home. Across the country, NHS England is providing funding to NHS trusts to ensure they all meet a core level of digitisation and have electronic patient record systems in place.
Will I be to access my hospital information through the NHS app?
Over time the plan is to integrate MY CARE with the NHS app so that you will be able to access your NHS related information in one place. Currently we are at the very early stages of integration. When we go live with the EPR those registered with both MY CARE and the NHS app will be able to see some basic hospital appointment information on the NHS app but will not be able to see everything currently available on MY CARE. Your first point of reference should, therefore, be MY CARE at this time and we will keep you updated should more functions become available on the NHS app.
Will MY CARE be safe to use?
Absolutely. MY CARE uses secure encryption to protect your personal health information, and we take all appropriate measures to comply with our data protection responsibilities to safeguard your data. Only you, your approved proxies and your approved healthcare providers can access your records.
When will the Trusts be able to access my information?
From January 2026, records from Torbay and South Devon (TSDFT) and University Hospitals Plymouth (UHP) will start to move to the new EPR and staff with access to this system may begin to have access to your records. This is to make sure your records are accurate and ready for the go-live dates: April 2026 for TSDFT and July 2026 for UHP. The Trusts have entered into a data sharing and processing agreement which sets out the legal framework for this sharing. Our privacy notice explains how we handle your information in our shared environments and how we will ensure your rights are respected.
