Do not run out of tablets. Always get a new prescription well in advance and keep plenty of supply at home
Home Delivery Service
This is a service offered to all patients through Lloyds Homecare Pharmacy.
3 monthly prescriptions can be requested via the pharmacist in clinic or via the contact details included on this leaflet. D
eliveries can be made to your home or a more convenient place as long as somebody is able to sign for the medications.
Please note:
- Prescriptions are not automatically generated- they MUST be requested from the hospital pharmacist and not Lloyds.
- Deliveries take at least 5 WORKING days so please ensure you have 2-3 weeks supply left when ordering.
Obtaining immunosuppression:
NHS England has determined that supply of immunosuppressant medication should be from the hospital’s transplant clinic rather than the GP surgeries. This Includes:
- Tacrolimus (Adoport, Prograf, Advagraf and Envarsus).
- Ciclosporin (Neoral).
- Sirolimus (Rapamune).
- Mycophenolate Mofetil.
- Mycophenolic Acid (Ceptava, Myfortic).
- Azathioprine
- Prednisolone.
- Valganciclovir.
Further supply should be obtained in clinic by speaking to the clinic based pharmacist, often in room 10 on clinic days. You can either collect your medications from the hospital or home deliveries can be organised.
Please ensure before each appointment you have enough medication to last until the next appointment
Medications on discharge from Hospital
On discharge from hospital you will be supplied with one month of ALL of your medications.
A copy of your updated medication list will be sent to your GP after you have been discharged.
You will have also been provided with a medication chart on the ward, this can be updated in clinic, please ask the pharmacist to do this.
How to Obtain Further Supplies all other medication (nonimmunosuppressant)
For all other medication that is not listed, please arrange further supplies from your GP.
