Our aim is...To provide alternatives to admission to hospital and facilitate discharge.

This leaflet is designed to answer some common queries you may have about our therapy service.

We are a team made up of Occupational Therapists (OTs), Physiotherapists and therapy

Support staff.

How we can help

  • Support to return to your home/care home safely

  • Assess for equipment

  • Support with your walking and activities of daily living

  • Support your family and carers

  • Give signposting information to help you once you have left hospital

  • Make referrals to other agencies and services

  • Reduce risk of falling

  • Advice on fracture management

  • Advice on managing musculoskeletal injuries

  • Advice on managing with respiratory conditions.

Physiotherapy

  • Specialist Musculoskeletal and Neurological assessments.

  • Mobility, falls, stairs and balance assessments.

  • Fitting neck collars, braces and slings.

  • Issue appropriate walking aids.

  • Respiratory condition management e.g. Breathlessness management, sputum clearance.

  • Exercise prescription.

  • Referrals on to other services, including community therapy.

Occupational Therapy

OTs wear a white top with green trim, and green trousers

  • Personal care and domestic activity assessments. Assessing how you manage getting washed and dressed and preparing hot drinks and snacks.

  • Transfer assessments. Assessing how you manage getting on and off different pieces of furniture like a chair, bed, bath or toilet.

  • Assessing any issues with memory or concentration that may impact on your daily living.

  • Order or provide equipment to help you at home. Arranging for equipment to be fitted to help you at home.

  • Refer or liaise with other agencies about your future needs.

  • Organise a package of care to support you at home or further therapy input if you need it.

  • Organise intermediate care support.
