‘Doing the rounds’ – a typical visit for a Specialist District Nurse

Many of the projects you have read about in this update will utilise a team of Livewell’s Specialist District Nurses to support patients in the community. This video follows Megan Bonner on a visit to a patient’s home.

David is a regular on Megan’s caseload, he had a history of falls resulting in a large skin tear on his arm. Megan was able to conduct a comprehensive geriatric assessment, which has helped to reduce his hospital attendance.

Megan Bonner “People have the mindset that we just go into patient’s houses, do a few dressings and give them insulin. Actually, we are faced with increasing complexity, avoiding hospital admissions and linking to the Acute GP and our Crisis Response Teams to be able to keep people at home.

“Although they are not emergency situations, they could be if we weren’t there.”
