Healthy Lives Partnership - Spring 2024 Update

Ian Lightley Welcome to the latest roundup from the Healthy Lives Partnership, the formal partnership between Livewell Southwest and University Hospitals Plymouth NHS Trust.

The theme for this update is  Attendance and Admission Avoidance and we’ve pulled together a series of stories from across the partnership - told by service users and staff.

As we move increasingly towards greater integrated care and partnership working between community and acute services, we have some great examples of driving forward improvements in access and experience, with updates on Virtual Ward, End of Life and Discharge to Assess, and important preventative activities, such as the support being offered to those at risk of falls (FAME) and frequent users of the Emergency Department (FUSE).

We have also forged a new model of Care Coordination with our Devon system partners that is showing great promise. We’ve been very supportive of this move in Plymouth with Livewell’s Urgent Community Response (UCR) Team being selected as the test-of-concept site ahead of the county-wide pilot. The model extends the great existing links between UCR, the Acute GP Service and Same Day Emergency Care (SDEC).
