We have introduced a new digital system for parents-to-be to register their pregnancy and access their health care records. Please bear with us while we transition to this new system.

My Pregnancy Notes

Maternity services in Plymouth are being reborn with the introduction of a new digital system for parents-to-be to register their pregnancy and access their health care records.  

The new system called My Pregnancy Notes is a fully electronic patient record, which supports the clinical and administrative management of your complete maternity journey. The new system goes live at Plymouth University Hospitals NHS Trust on Wednesday 22 November.  

My Pregnancy Notes benefits  

  • You will be able to register for an account when you discover you are pregnant, giving a streamlined referral to our services. 

  • We can share information quickly and easily between all people involved in your care, enabling the midwife to have more personalised conversations.  

  • You can record and view your appointments, access your pregnancy notes, and view leaflets and other information. 

  • You will have greater visibility, control, and access to trusted information. 

All information is held securely and cannot be accessed without the correct login details (like internet banking).   

Using My Pregnancy Notes 

 

You will be able to access your maternity notes and information through smart phones or other devices, improving your overall maternity experience.  

  • If you are already booked for pregnancy, we will move to the electronic system for ongoing care but ask that you continue to bring your paper notes to appointments. 

  • If you are letting us know about a new pregnancy from the end of November onwards your entire pregnancy journey will be on My Pregnancy Notes.  

If you are currently pregnant and having your baby with University Hospitals Plymouth, register for an account at My Pregnancy Notes selecting ‘Derriford Hospital’ as the site. 

If you are letting us know about a new pregnancy, you can still register for a My Pregnancy Notes account, but you must also register on our ‘Midwife Booking’ form so we can arrange your first appointment with a midwife.

  • Once you complete the form you will receive a reference number. If you do not receive a reference number, please re-submit the form.
  • Please be assured that this reference number means that we have been notified of your pregnancy and we will be in contact as soon as possible to organise your first appointment.
  • If you have not heard from us by the time you are 8 weeks pregnant, or you believe you are already over 8 weeks pregnant, please call 01752 437555.
