Champions for Elder Care

“Driving the Trust forward in improving care for the elderly, and those with dementia”

Our Champions for Elder Care are a multi-professional working group with user/carer involvement.

The group completed a comprehensive self assessment of care of patients with Dementia. A framework was followed, with 8 key standards:

• Respect, dignity and appropriate care
• Agreed assessment, admission, discharge processes and needs specific care plans
• Access to a specialist mental health liaison service
• Dementia-friendly environment and minimising moves
• Nutrition and hydration needs are well met
• Promote the contribution of volunteers and families
• Quality of care at the end of life
• Appropriate training and workforce development
