What do we do?
We educate and empower families to meet their children’s complex health needs. We also support with the delivery of nurse-led care and treatment.
We offer support for a wide range of acute referrals as well as chronic, complex and end of life diagnoses.
We have strong links with a wide range of professionals working within health, education, and social care settings.
We aim to keep children and young people well and at home. We also strive to facilitate safe and early discharge if hospital admission is required.
We work closely with other agencies to support packages of care in the home setting.
We provide families with equipment and/or supplies via a community order.
If you are provided with equipment by the CCN team, this is jointly owned by University Hospitals Plymouth NHS Trust and the commissioning board for Devon. It is your responsibility to ensure that the equipment is only used as intended and that it is kept clean and regularly maintained.
We ask that families check the equipment before each use, store it carefully to prevent children playing with it and take care when transporting to prevent damage.
Community orders will need to be collected by families from the Child Development Centre.
The service operates 9am to 5pm, Monday to Friday.
We aim to monitor our phone line throughout the day but our voicemail is checked daily at 9am and 1pm.

Unfortunately, we are unable to process requests for medications. Please liaise with your named GP, Consultant or Pharmacy regarding this.

We are a group of specialist children’s healthcare professionals working across Plymouth as well as parts of Cornwall and Devon to provide community nursing services to children aged 0 to 18 years.
