Make an Autism referral

Derriford Autism Service requests that if anyone has contact or knowledge of patients - adults or children - who are Autistic (either with a formal diagnosis or those who know that they are autistic but do not wish to go down the diagnostic pathway), that they inform us of the names/NHS numbers etc. so that we can add the appropriate alerts to the hospital systems.  There are four systems that we currently place alerts on, and placing an alert yourself on any one of those will not enable us to ensure they are alerted on the other systems.

There is no formal register anywhere and although we know that there are potentially 10,000 Autistic people in our catchment area we have significantly less alerted. 

Make a referral

Please let us know the following details when you email or call:

• Name

• Address

• NHS number (if known)

• Hospital number (if known)

• If you have a formal diagnosis of Autism/Aspergers

• If you know or think you have Autism/Asperger’s but haven’t been told officially
