Patients Know Best (PKB)

We are pleased to offer our patients a new online service that gives you secure access to your health record. It’s designed to improve your patient experience and access to NHS services and information.

All your data is stored securely. Simply log in to view your healthcare information or use it to understand and monitor your condition.

This service is free to our patients and is offered using an NHS accredited system called Patients Know Best (PKB). It’s part of our promise and ongoing commitment to give you more choice and control over your care. The service will allow you to:

  • See all your appointments, medical correspondence, test results, medication lists and care plans, together in one place, wherever you go.
  • Receive notifications when new information is available, prepare any questions ahead of appointments and manage your health and care in the meantime.
  • Add and organise your health information
  • Add important information about your past medical history or any old clinical documentation so it’s in one place, when you need it.
  • Upload old reports, care plans or test results and store them securely online, rather than carry around any old bits of paper or files.
  • Track your health
  • Keep a journal, monitor your symptoms and add measurements.
  • Share this data with healthcare teams from the comfort of your own home in real-time.

Not all services are using PKB this is because it is not suitable for all types of patients pathways. However, If your clinical team in University Hospitals Plymouth NHS Trust  XXXXXXXXXX they will contact you to discuss.

Patients know best (PKB) Privacy Notice

Some areas in the Trust use a patient portal called Patients know best (PKB).

We use this system to enable patients to self-manage long term conditions and give them greater control over their healthcare. This is linked to how the Trust manages some patients pathways, which is known as Patient Initiated Follow Up (PIFU).

PKB is a method for allowing patients to have direct access to their healthcare record in real time. Whilst the patient and the clinician can view the information, this is not visible to PKB.

If your clinical team is using PKB they will contact you to discuss.
