We can found on Level 6 in the Diabetes Centre (opposite Outpatients)                                

We provide specialist diabetes advice, support and education to patients, carers and all staff within the hospital.

We are part of a multidisciplinary team which include Diabetologists, Specalist Registrars, Specialist nurses and Diabetes Support Assistants.

We aim to assess and review patients who are referred internally via seeEHR forms. These include patients who have uncontrolled diabetes, new diabetes diagnoses, diabetes emergencies and patients who self-refer.

We liaise closely with the community diabetes teams, GPs, and initially follow up patients after discharge if required.

We pride ourselves on giving a patient centred holistic approach aiming for high quality diabetes care.
