MY CARE for Children, Young People and Families
MY CARE is the secure patient portal used by University Hospitals Plymouth NHS Trust. It allows patients and families to access important hospital information in one place.
Why sign up?
By registering for MY CARE you can view key information about your child's care, including:
- Upcoming and recent hospital appointments
- Clinic letters
- Test results
- Communication from your healthcare team
- Access information about your child's health from your computer, tablet or smartphone
MY CARE for children and young people
Children under the age of 16 do not usually have access to their own MY CARE account. Instead a parent or guardian can request proxy access, allowing them to manage their child's healthcare information. To request proxy access, the parent or guardian must first have an account of their own.
In some circumstances, children aged 12 to 15 may be granted their own access to MY CARE. This is assessed on an individual basis and will be discussed with the clinical team, taking into account the young person's needs, circumstances and ability to manage their own healthcare information.
How do I get proxy access?
If you would like access to your child's MY CARE record, please speak to your child's clinical team at your next appointment.
When a young person turns 16, the parent or guardian proxy access is automatically removed. However, they can chose to grant you access again through their own account.
If the young person is unable to provide access themselves due to their health or circumstances, their healthcare team may be able to arrange proxy access at their next appointment.
Getting started
If you are eligible to use MY CARE, you can access it through the MY CARE website or MyChart app using your NHS login or creating a MY CARE account.
