Early Supported Discharge Team (ESD)
Patients who leave the Acute Stroke Unit at Derriford Hospital or the Stroke Rehab Unit at Mount Gould Hospital and require ongoing therapy, may be referred to the Early Supported Discharge Team, depending on whether they meet the referral criteria.
This is a team of stroke specialist Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists and Therapy Support Workers.

For patients who do not meet the criteria for the ESD team, but still require ongoing rehabilitation, there are other community therapy teams which we can refer into.
