To register a new pregnancy with us, please fill out the below form and a member of the maternity team will contact you:

If you have not heard from us by the time you are 8 weeks pregnant, or you believe you are already over 8 weeks pregnant, please call our community midwifery appointment team on 01752 433256.
Your Maternity app is changing
From 23 July 2026, My Pregnancy Notes (MPN) will no longer be used for your current pregnancy. Instead, you'll use MY CARE to access your maternity information.
Your maternity team, appointments, care plan and place of birth plans all stay the same. The only change is the online system used to view your maternity information.
Why are we making this change?
We are introducing a new electronic patient record across University Hospitals Plymouth NHS Trust. This means your maternity information will be available through MY CARE, giving you more access to your records and improving how information is shared between healthcare professionals involved in your care.
What can I do in MY CARE?

You can access MY CARE securely from your phone, tablet or computer, 24 hours a day.
What happens to My Pregnancy Notes?
From 23 July 2026, your current pregnancy in My Pregnancy Notes will show as closed and will no longer receive updates. All information about your current pregnancy will be available in MY CARE.
Information from previous pregnancies will still be available in My Pregnancy Notes.
How do I sign up?
On a computer:
Visit MY CARE and follow the on-screen instructions.
On a smartphone or tablet:

Download the MyChart app from the Apple App Store or Google Play Store.

Open the app.

Search for your local hospital trust: University Hospitals Plymouth NHS Trust

Select MY CARE and follow the sign-up instructions.
Once you have signed up, you can continue to access information about your pregnancy online.
